  • Public Announcement Concerning Charlene Manor Extended Care Facility, 130 Colrain Road, Greenfield, MA 01301

    October 6, 2022 – Greenfield Management Systems, Inc., 75 North Street, Ste. 210, Pittsfield, MA 01201 (the “Buyer”) has submitted an application to the Department of Public Health, Division of Health Care Facility Licensure and Certification (the “Department”) for the proposed change in ownership of Charlene Manor Extended Care Facility, 130 Colrain Road, Greenfield, MA 01301 (the “Facility”).  The Buyer currently leases the Facility from, and manages the Facility on behalf of, the current owner, Charlene Manor Nursing, LLC, c/o Chakalos Management, LLC, 135 South Road Farmington, CT 06032 (the “Seller”). The Buyer, if deemed suitable by the Department, intends to purchase the Facility from the Seller and will continue to manage and operate the Facility after the change in ownership. There will be no changes in the bed capacity or the services provided by the Buyer at the Facility. A public hearing may be requested upon a petition by any group of ten adults.  Such petition shall include the name, address and signature of each adult.  Written comments concerning the Buyer’s ability to provide quality long-term care services and petitions for a public hearing may be addressed to the Department of Public Health, Division of Health Care Facility Licensure and Certification, 67 Forest Street, Marlborough, MA 01752, or [email protected], for a period of 14 days following the publication of this notice.  All written or oral comments submitted to the Department may be posted on the Department’s website and released in response to a request for public records.
